Upload Standardized Test Score Reports

Please upload your child's 5th and 6th grade standardized test scores, such as PARCC, NJSLA, Start Strong, Terra Nova and Iowa CogAT, ERB test, SSAT/ISEE, PSAT, SAT, etc. Alternatively, you can bring the hard copies with you to the office on the appointment date.


Extracurricular Activities and Accolades

List your extracurricular activities you have participated in, such as school clubs, student government, sports, community service, musical instruments, and arts as well as accolades you have received. Number each activity or accolade. Enter 'none' if there are none.
